Mount Koryphe in Argolis (there's two parts to this area) Kradia Isle under Samos. Chytra Atoll under Kythera Island. Valley . It isn’t a bug. I didn't unlock it until my second playthrough, thought I had found everything on my first but a tiny little section near where you start the daughters of artemis quest is what finally unlocked it for me. Not bugged. It counts most of the tiny little islands, every land and sea section.
The Hermes Homie trophy is glitched. Went to all the regions, removed all the fog of war on both land and ocean, found all sync points and the trophy still isn't popping up. It's glitched for sure. I had a small bit of ocean that I hadn't visited that was the reason mine didn't clear. Even though I'd explored every island and land. 3. Without screenshots it's just kinda us guessing. The two I missed were Valley Of The Kings in Lakonia and the little island off the bottom of Kythera Island at the very bottom of the map that counts as a separate region. DLC isn’t factored in for this achievement.

I've completed the game and my "Regions Discovered" progression says 100%, but no Hermes' homie achievement. I've defogged the entire map, been on every single island, even the little below Samos.
